लड़कियों में autism — क्यों सालों तक miss होता है, और क्या देखें
हर चार लड़कों पर जिन्हें autism spectrum disorder formally diagnose होता है, लगभग एक लड़की होती है। यह ratio, U.S. CDC के latest surveillance data से, दशकों से steady है। Researchers जो अब इस number को क़रीब से देख रहे हैं, उन्हें suspect है कि इस gap का बड़ा हिस्सा biological difference नहीं है — यह diagnostic difference है। Autism वाली लड़कियाँ differently present करती हैं, अपने traits ज़्यादा skilfully छिपाती हैं, और overlooked हो जाती हैं उन तरीकों से जिनसे लड़के usually नहीं होते।
Consequence सिर्फ़ late diagnosis नहीं है। यह सालों तक एक लड़की का different feel करना, खुद को blame करना, छिपाने की कोशिश करना, और अक्सर anxiety, depression, या eating problems develop करना है — उस underlying autism के ऊपर जो अभी तक किसी ने नाम नहीं दिया।
मैं यह लेख उस parent के लिए लिख रही हूँ जिसने autism की बढ़ती rates पर हमारा पिछला लेख पढ़ा और सोचा "पर मेरी बेटी उन autistic लड़कों जैसी नहीं लगती जो मैंने देखे हैं।" वो अभी भी autistic हो सकती है। मुझे दिखाने दें क्या देखना है।
Numbers likely झूठे क्यों हैं
4-to-1 boy-to-girl diagnostic ratio formal diagnosis records से आता है, population में prevalence से नहीं। जब researchers पूरे school populations को blindly screen करते हैं — हर बच्चे से same questions पूछते हैं, सिर्फ़ referred वालों को evaluate करने के बजाय — तो ratio dramatically narrow हो जाता है। UK और Australia की studies suggest करती हैं कि true ratio 2-to-1 या 3-to-2 के भी करीब हो सकता है।
Missing girls कहाँ जाती हैं? तीन जगहें, mostly:
- Referred ही नहीं होतीं। उनके teachers, parents, या paediatricians signs को नहीं पहचानते क्योंकि "लड़कियाँ वैसी नहीं दिखतीं।"
- Referred पर diagnose नहीं होतीं। वो assessment के दौरान अच्छे से mask करती हैं। Clinician एक shy पर sociable लड़की देखता है और autism rule out कर देता है।
- Misdiagnosed। जो label उन्हें eventually मिलता है वो है anxiety, depression, eating disorder, "shy," "sensitive," या "highly gifted" — इनमें से सब autism के साथ co-exist कर सकते हैं पर उसे replace नहीं करते।
जब तक ये लड़कियाँ adolescence तक पहुँचती हैं, लगभग आधी अपनी eventual autism diagnosis mental health crisis के context में receive करती हैं। यह एक preventable tragedy है। पहले diagnose हुई होती, तो चीज़ें टूटने से पहले उन्हें support मिल सकती थी।
लड़कियाँ differently क्यों present करती हैं
Researchers ने तीन overlapping reasons identify की हैं:
Different neurological patterns। Emerging brain-imaging research suggest करती है कि autistic लड़कियाँ social processing के लिए autistic लड़कों से अलग brain regions use करती हैं। उनका mirror neuron system — वो हिस्से जो दूसरे लोगों के expressions pick करते हैं और copy करते हैं — ज़्यादा actively काम करता दिखता है। इससे surface-level social behaviour उनके लिए easier होता है।
Different social expectations different behaviours shape करती हैं। बहुत छोटी उम्र से, लड़कियों को लड़कों से ज़्यादा direct social coaching मिलती है — "thank you कहो", "auntie को smile दो", "बहन के साथ share करो"। ये external scripts internalise हो जाती हैं। एक autistic लड़की अक्सर correct social behaviour intellectually जानती है — भले intuitively feel न करती हो — और perform करती है। इसे "camouflaging" या "masking" कहते हैं।
Different special interests। Autistic लड़कों के intense interests अक्सर visibly unusual होते हैं — train timetables memorise करना, dinosaurs का cataloguing, vehicles का obsession। Autistic लड़कियों के interests अक्सर वो चीज़ें होती हैं जो लड़कियों को "पसंद होनी चाहिए" — घोड़े, dolls, एक specific book series, K-pop, जानवर — बस उस intensity से pursue की जाती हैं जो peers से बहुत आगे जाती है। क्योंकि topic normal दिखता है, intensity miss हो जाती है।
एक लड़की में autism असल में कैसा दिखता है
यहाँ वो patterns हैं जो मैंने clinic में सबसे ज़्यादा देखे हैं। सब हर एक लड़की पर apply नहीं होंगे। अगर कई सच लगते हैं, तो proper evaluation worth है।
Social presentation
- एक बहुत close friend है, group नहीं। अक्सर खुद से बहुत बड़ी या बहुत छोटी। उस एक से आगे किसी friendship में struggle कर सकती है।
- School में group के edge पर। दूसरी लड़कियों के साथ-साथ खेलती है पर पूरी तरह join होना नहीं जानती।
- दूसरी लड़कियों का behaviour intentionally copy करती है। ध्यान से देखती है, फिर rehearse करती है क्या कहना और करना है। यह masking है — real है, fake नहीं, पर exhausting।
- School से घर आकर collapse हो जाती है। दिन भर hold कर सकती है, फिर उसी moment गिर जाती है जब वो safe हो। शाम में meltdowns, extreme irritability, या पूरी withdrawal।
- Unstructured social time में struggle। School lunches, birthday parties, और free play structured classroom time से बहुत ज़्यादा मुश्किल हैं।
- Adults के साथ one-on-one confident और articulate लग सकती है, पर groups में quiet या lost।
- Adolescence में अचानक social difficulties जब social interaction के rules ज़्यादा complex और कम scripted हो जाते हैं।
Sensory patterns
- Clothes के बारे में बहुत picky। Tags, seams, specific fabrics से मना; exact same clothes बार-बार चाहिए।
- Noises, crowds, या smells के लिए sensitive जो proportion से बाहर लगती है। Noisy environments में shut down हो सकती है या anxious।
- Food के बारे में बहुत picky। Textures, temperatures, या brands — अक्सर narrow diets जो समय के साथ सिकुड़ते हैं।
- Physical touch complicated है। Deep pressure hugs crave कर सकती है पर light touch से नफ़रत; बालों में कंघी, brush, या hug resist कर सकती है।
- Small changes notice करती है। एक moved cushion, अलग smell, नए person की आवाज़।
Interests और rigidity
- Deep, focused interests जो peers से ज़्यादा intense लगते हैं। वो सिर्फ़ horses पसंद नहीं करती — वो breeds memorise करती है, constantly draw करती है, और किसी और चीज़ के बारे में बात नहीं कर सकती।
- Change में difficulty। Teacher बदलना, schedule shift, friend का move करना — सब disproportionate distress देते हैं।
- घर पर strong routines। Meals particular order में होने चाहिए, bedtime rituals vary नहीं कर सकते, weekend plans को long notice चाहिए।
- Repetitive play जो peers के आगे बढ़ जाने के बाद भी चलती है। Dolls line up करती है, toys को colour से arrange करती है, same movie 100 बार देखती है।
Emotional patterns
- Emotions intensely आती हैं और settle होने में लंबा समय लेती हैं। मिनटों में zero से overwhelm, घंटों या दिनों में baseline पर वापस।
- Trigger से बहुत बड़े meltdowns। एक छोटी frustration एक बड़ा तूफ़ान लाती है। ख़ासकर घर पर, school में कम।
- Perfectionism paralysis तक। Task शुरू नहीं कर सकती अगर perfectly नहीं कर सकती। खुद का work फाड़ देती है।
- Anxiety, ख़ासकर social situations के बारे में। Event से दिनों पहले anticipatory worry।
- Adolescence में identity struggle — different feel करती है, नहीं जानती क्यों, अक्सर खुद को blame करती है।
Academic patterns
- बहुत academic हो सकती है — कुछ autistic लड़कियाँ early readers होती हैं, आसानी से memorise करती हैं, structured subjects में बहुत अच्छा करती हैं।
- Rules से प्यार। Clear structure चाहती है, दूसरे rules तोड़ें तो upset।
- Group projects या open-ended assignments में difficulty।
- Uneven skills profile — कुछ चीज़ों में excellent और unexpectedly दूसरों में poor।
Specifically Indian complications
Indian family और school culture लड़कियों के autism को और easier बनाती है miss करना:
"वो बस shy है" लड़कियों के लिए ज़्यादा culturally acceptable है। एक quiet, well-behaved लड़की जो अपना homework करती है और class disrupt नहीं करती वो "अच्छी लड़की" है, ऐसी लड़की नहीं जिसे मदद चाहिए हो।
लड़कियों से socially skilled होने की expect होती है जिस तरह लड़कों से नहीं। Autistic लड़कियाँ इस expectation को पूरा करने में massive effort डालती हैं। घर पर उनकी exhaustion tantrums या moodiness के तौर पर misread होती है, दिन भर की masking से burnout के तौर पर नहीं।
Perfectionism praised होती है। एक autistic लड़की की चीज़ों के exactly right होने की need अक्सर excellent academic results produce करती है। Parents और teachers results celebrate करते हैं। नीचे का distress unseen जाता है।
Female friendships भारत में highly scripted हैं। एक autistic लड़की script को इतना अच्छा follow कर सकती है कि "friends हों" — बिना उन friendships में हो रहे emotional exchange को कभी really समझे। वो फ़र्क feel करती है। उसके parents shayad ही देखते हैं।
Adolescence वो है जब पहिये निकल जाते हैं। Age 12-14 के आसपास, social rules ज़्यादा implicit हो जाते हैं, कम scripted। Menstrual hormones उसकी पहले से fragile emotional regulation को destabilise करते हैं। Academic pressure बढ़ता है। यह वो है जब कई Indian families को पहली बार realise होता है कुछ ग़लत है — अक्सर anxiety, depression, या "board exam stress" label हो जाता है।
क्या co-occur करता है, और क्यों मायने रखता है
लड़कियों में autism अक्सर दूसरी conditions के साथ आता है। इन patterns की जानकारी underlying autism को spot करने में मदद कर सकती है:
- Anxiety disorders — extremely common; अक्सर presenting complaint
- Depression, ख़ासकर adolescence में जब masking exhausting हो जाती है
- Eating disorders, ख़ासकर avoidant/restrictive food intake या anorexia — food के आसपास sensory rigidity और control की need
- Selective mutism — घर पर freely बात, कुछ settings में silent
- ADHD — अक्सर co-occur; लड़कियाँ hyperactive से ज़्यादा inattentive type present कर सकती हैं
- Chronic stomach problems, headaches, या fatigue बिना clear medical cause — शरीर वो carry कर रहा जो मन express नहीं कर सकता
अगर आपकी बेटी का anxiety या depression के लिए treatment हुआ है पर treatment काम नहीं कर रहा, या अगर उसे eating disorder है जो standard approaches पर resist करता है, तो पूछना worth है कि क्या autism consider किया गया है।
अगर आप suspect करते हैं तो क्या करें
अगर यह पढ़कर आपकी बेटी के बारे में concerns उठे हैं, तो practical path यहाँ है:
Observation से शुरू करें, accusation से नहीं। उसे कुछ हफ़्तों तक देखें। Patterns note करें। क्या meltdowns specific triggers के बाद होते हैं? School में और घर पर वो कैसी होती है, इसमें फ़र्क है? उसकी sensory preferences क्या हैं? लिखें।
उससे बात करें, gently। बड़ी लड़कियाँ अक्सर पहले से जानती हैं कि कुछ different है — बस उनके पास language नहीं है। एक conversation जो शुरू होती है "क्या तुमने कभी feel किया कि दूसरे लोग ऐसे rules follow कर रहे हैं जो तुम्हें दिखते नहीं?" दरवाज़े खोलती है। Diagnostic नहीं — supportive।
Proper evaluation seek करें। General paediatrician की opinion नहीं। आप एक developmental paediatrician, paediatric psychologist, या child psychiatrist चाहते हैं जिसके पास specifically लड़कियों में autism का experience हो। अगर पहला clinician जिसे आप देखें वो बिना full assessment के आपके concerns dismiss करता है, तो second opinion seek करें — यह specific dismissal सबसे common mistakes में से एक है।
Evaluation के लिए तैयार रहें। अपने written observations लाएँ। उसके intense interests, sensory patterns, घर पर meltdowns के examples लाएँ। Clinicians उसे एक घंटे के लिए देखते हैं; आप उसे पूरे हफ़्ते देखती हैं। आपका data मायने रखता है।
Diagnostic tools मायने रखते हैं। पूछें कि क्या clinician ADOS-2 (female presentation के लिए updated) use करता है, आपके साथ interviews के साथ। Tools के पुराने versions लड़कियों का presentation miss करते हैं।
Diagnosis life sentence नहीं है — यह starting point है
Indian parents से मैं जो fear सबसे ज़्यादा सुनती हूँ: "पर अगर उसे diagnosis मिली, तो उसके marriage prospects / career / opportunities बर्बाद हो जाएँगे।"
मैं यह डर समझती हूँ। यह misplaced है। Diagnosis एक private medical record है। इसे disclose करने की ज़रूरत नहीं है। जो यह बदलती है वो है क्या आप और वो अपने बारे में जानती हैं — और इसलिए उसे क्या support मिलती है, वो school में क्या accommodations access कर सकती है, उसके लिए क्या strategies काम करती हैं, और वो खुद को blame करना क्या रोक सकती है।
जो लड़कियाँ बचपन या early adolescence में autism diagnosis पाती हैं, supportive parents के साथ, वो adulthood में उनसे dramatically better करती हैं जो 25 पर mental health crisis के बीच में diagnose होती हैं। Intervention window उम्र के साथ बंद होता जाता है। बिना support के हर साल की masking उसे कुछ costs करती है।
एक line याद रखने लायक
